Special Dietary Needs

If your child needs a change to their meals because of a disability or medical condition, we will make it work. Please have a medical authority complete our Special Diet Statement so our kitchen can safely provide the right food. The form can be submitted to the Health Office by mail or in person: HOPE Community Academy, 720 Payne Ave, Saint Paul, MN 55130, by fax: 651-972-8481, or email: suemoua@hope-school.org.
